Community intake

One clear front door for ideas, experience, and concerns.

Choose the community involved, then use the appropriate form to share an idea, describe an experience, or report a factual concern for private staff review.

Step 1 · Select the community or service

Make a suggestion

Use this for a constructive idea, a problem worth solving, or an opportunity the selected community should consider. A strong submission explains the need as well as the proposed change.

Options update with the selected community.

Optional supporting files

Up to three files: .txt logs or JPEG images only. Each file may be up to 1 MB, with a 2 MB total. Files are stored privately and are available only to authorized reviewers.

Submission creates a reference number. Reports are not monitored in real time and no particular action or response time is promised.
